JATENZO – 237mg
JATENZO (Testosterone Undecanoate) 237 mg capsules are an FDA-approved oral androgen medication used for testosterone replacement therapy (TRT) in adult men with hypogonadism due to structural or genetic causes, such as disorders of the testicles, pituitary gland, or hypothalamus, leading to low or absent endogenous testosterone production. Symptoms of low testosterone may include reduced libido, fatigue, erectile dysfunction, decreased muscle mass, mood changes, and infertility. The 120-capsule pack provides a supply for typical dosing over about 2 months.
How It Works
JATENZO is a prodrug consisting of testosterone esterified with undecanoic acid, forming a lipophilic compound absorbed via intestinal lymphatics into the bloodstream, bypassing first-pass liver metabolism and reducing hepatotoxicity compared to older oral testosterone forms. After oral administration with food (to enhance bioavailability), it undergoes hydrolysis by esterases to release active testosterone, which binds to androgen receptors to promote protein synthesis, nitrogen retention, red blood cell production, and maintenance of male secondary sexual characteristics, restoring serum levels to physiologic ranges.
Dosage and Administration
- Available in 158 mg, 198 mg, and 237 mg softgel capsules; the 237 mg strength is opaque orange and imprinted with “237”.
- Recommended starting dose: 237 mg orally twice daily (morning and evening), with food containing 15-45 g fat to optimize absorption.
- Adjust dose based on serum testosterone levels measured 6 hours after the morning dose (e.g., increase to 316 mg BID if low, decrease to 158 mg BID if high); maximum 396 mg twice daily.
- Confirm hypogonadism diagnosis with morning testosterone tests on two separate days before starting; monitor levels periodically and adjust as needed.
- For the 120-capsule pack: Take consistently; do not crush or chew capsules. Regular blood tests for testosterone, hematocrit, PSA, lipids, and blood pressure are essential.
Side Effects
Common side effects include:
- Increased hematocrit (red blood cell count), headache, hypertension, diarrhea, indigestion, burping, nausea, leg/foot swelling, and enlarged prostate.
- Androgenic effects: Acne, mood changes, or aggression.
Serious risks: Blood pressure elevation (may increase major adverse cardiovascular events like heart attack or stroke, especially in those with CVD risk factors), polycythemia (thicker blood increasing clot risk), prostate issues (worsening BPH or cancer risk), gynecomastia, sleep apnea exacerbation, edema, or mood disorders including depression/suicidal thoughts. Monitor blood pressure regularly; discontinue if severe hypertension or cardiovascular symptoms occur and seek immediate medical help. Unlike injectable forms, oral JATENZO avoids pulmonary oil microembolism but still requires caution.
Precautions and Warnings
- Contraindicated in men with breast/prostate cancer (known or suspected), pregnancy (causes fetal virilization), hypersensitivity to ingredients, or non-structural/genetic hypogonadism (e.g., age-related).
- BOXED WARNING: Increases blood pressure; monitor periodically, especially in hypertensives or those with CVD risks—may need discontinuation if uncontrolled.
- Not for women, children, or abuse (e.g., bodybuilding); controlled substance (CIII) with abuse potential leading to cardiovascular/psychiatric harm.
- Use caution in BPH, sleep apnea, diabetes (may alter glucose/insulin needs), or polycythemia; evaluate prostate before and during therapy.
- Interactions: May potentiate anticoagulants (e.g., warfarin—increased bleeding risk), insulin (hypoglycemia), antihypertensives (reduced efficacy), or BP-raising meds (e.g., NSAIDs, stimulants); 191 known drug interactions—inform doctor of all medications/supplements. Take with food; avoid alcohol excess.
